Kalida Local Schools

All students must have a physical on file before they begin practicing with any athletic team. Kalida High School will NOT be providing free physicals to students entering grades 7 and 8 for the upcoming year.  These students will have to obtain a physical from their family doctor.  

An alternative choice would be a FREE physical on Monday, May 2, or May 9, from 5:30-8:00 p.m. at the Orthopaedic Institute of Ohio (OIO) located at 801 Medical Drive, Suite A, Lima.    Students going to OIO must complete all paperwork in the packet and make sure to fill out the Consent for Pre-Participation Physical Form before going to OIO.  

FREE physical night for those entering grades 9-12 is Monday, May 23, from 6-8:00 p.m. in the new gymnasium.  If a student wears corrective lenses/contacts, please bring or wear them to the physical in order to complete the eye exam.  Pick up a packet of forms from the high school office for the physical examination.  All forms must be completely filled out before you receive a physical.
