2024-2025 OHSAA PPE Physical (For Physicians) Form
From the Athletic Department: The OHSAA requires that athletes have a new school physical on file for each year. Physicals for next year cannot be completed until after May 1st of the current year.
From the Kalida Medical Arts Staff: To ALL parents of student athletes. Dr. Klir will be doing sports physicals on Saturday, May 11th from 9:00 a.m.till 12:00 noon. Also on Thursday, May 30th, Melissa will be doing the sports physical from 1:00 p.m. till 7:30 p.m.. Students will need to call beforehand, 419-532-3959, to have their sports physical scheduled and on the day of their appointment, they will need to bring their form already completed.
