Athletic Physicals for the 2019-2020 School Year

All students must have a physical on file before they begin practicing with any athletic team. A student needs to obtain a new physical each year. All physicals for next school year must take place after May 1, 2019 to be valid. Kalida High School will be providing FREE physicals for students entering grades 9-12 who are planning to play interscholastic sports during the 2019-2020 school year. The Physical Packets will be given to the students. The packet of forms must be completely filled out before you receive a physical. FREE physical night for those entering grades 9-12 is Monday, May 20, 2019 from 6:00-7:30 p.m. in the new gymnasium. If a student wears corrective lenses/contacts, please bring or wear them to the physical in order to complete the eye exam. Dr. Horstman and Dr. Klir will be on hand to do physicals for our students free of charge. This is the only date possible for a free physical. Please plan to attend.

Note: Kalida High School will NOT be providing free physicals to students entering grades 7 and 8 for the upcoming year. These students will have to obtain a physical from their family doctor. An alternative choice would be a FREE physical on Monday, May 6, or Monday, May 13, 2019 from 5:30-7:30 p.m. at the Orthopaedic Institute of Ohio (OIO) located at 801 Medical Drive, Suite A, Lima, OH 45804. Students going to the Orthopaedic Institute of Ohio will need to bring the entire packet with them. Make sure to fill out the Consent for Pre-Participation Physical Form and the Patient Health Questionnaire in the packet in order to obtain the physical at OIO. After the physical exam is completed, OIO will send the completed packet back to the school. For those students obtaining a physical from their family doctor, the student will have to bring the completed physical packet back to the school. Note: All physical packets have been sent home with the students.
